CCP Proof of Counseling for the 2022-2023 School Year (Covington Schools)
Covington Schools is required to have documentation showing that each student AND parent interested in participating in the College Credit Plus program has received counseling. Once you have completed the steps on the posted Instruction Sheet, complete and submit the form below (one of each student) so your participation can be documented. The student and parent should complete this form together.
