2019 Indiana Philanthropy Awards Nomination Form: Association of Fundraising Professionals-Indiana Chapter (AFP-IC)
Nominations for 2019 Indiana Philanthropy Awards are being accepted May 1 through June 17, 2019. Award categories and nomination instructions are detailed on the AFP-IC website at: bit.ly/IPAwards.

Nominations must include this completed nomination form and two letters of support (maximum of one page each) emailed to AFPAwards@gmail.com. Please expect an update about the status of your nomination by mid-August.

Form notes:
- This form must be completed at one time, as partially-completed forms cannot be saved.
- All categories marked "required" must be complete.
- You will immediately receive an email confirmation at the email address provided below once this form is successfully submitted.
- Nominators do not need to be AFP-IC members. Nominations are open to anyone.
- Nominees must reside in Indiana.

Awardees will be honored at the 37th annual Indiana Philanthropy Awards Celebration. Honorees are required to attend the celebration and nominators are strongly encouraged to attend. The awards committee will communicate directly with honorees in preparation for the celebration.

Please include three to five paragraphs (averaging a total of 400-600 words) summarizing the rationale for this nomination below. This should include: 1. Description of outcomes of philanthropic impact and achievements accomplished. 2. Evidence of philanthropic support and innovative, collaborative approaches to solving problems. 3. For Outstanding Fundraising Professional, Philanthropist, Volunteer Fundraiser, and Lifetime Achievement Categories only, also include evidence of encouragement and motivation given to others to take leadership roles toward philanthropy and community involvement.
