1. Please note that this document is not a comprehensive list of all district policies, and that these summaries are not a substitute for the policies themselves. Full copies of all of the policies referenced in this handbook are available online at www.wcpss.net/handbook, at the school system’s central administrative offices at 5625 Dillard Drive in Cary, and at the front office of every school. All other policies and procedures are available online at www.wcpss.net/policy and at central office. Students and their families are urged to review the actual language of these policies and procedures, which include important information about your rights and responsibilities. By signing and returning this page, you acknowledge that you have received this handbook. This form will be kept at your child’s school. WCPSS Policy and Forms (Part Two): The following forms must be completed on paper and are linked below. You may submit via email; email forms to crossroadsflex@wcpss.net (email subject line: Forms for {Student Name}).                                                                                                            *Please note, forms 3, 4, and 5 only need to be completed upon a child's first year in WCPSS and/or if the parent/guardian wishes to update information.
If your child has previously been enrolled in WCPSS and your information is current, you may click "Next" after completing the Student Device Program Agreement.
2. Student Device Program Agreement
ALL families should complete this form, indicating their selection to use or not use a district provided device.

3. Student Name/Photo Privacy Release
Please complete this form and have your student return it to his or her school. This consent form remains valid throughout your student’s K-12 experience with the Wake County Public School System or until a new form is completed and signed by a parent/guardian or eligible student.

Please note: This form is not required for returning students unless they wish to change their current photo/privacy release information already on file at the school.

4. Consent for Technology
This consent form remains valid throughout your student’s K-12 experience with the Wake County Public School System or until a new form is completed and signed by a parent/guardian or eligible student.

Please note: This form is not required for returning students unless they wish to change their current technology consent information already on file at the school.

5. Military Connected Students
Please submit if an immediate family member of your child connected to the U.S. Military, including Active Duty, National Guard or Reserves, Retired Military, Disabled Veteran or a Federal Civil Service Employee.
