Before submitting this form, be sure to download our Mobile App (IMPACT ACTIVITIES TX)  and create your account if you are not already in our system.
After creating your account, complete this online form and our associates will call you to complete the registration.

Notes:
  • Memberships dues are charged automatically on the 1st of each month using EFT (Electronic Fund Transfer).
  • Members are required to check-in at the front desk by scanning the member ID barcode (Card or Electronic version on the APP)
  • Impact Activities does not offer (On-Hold) option for memberships however members are allowed to cancel at any time with a 30-day notice. (please note that full initiation fee is charged upon re-registration for everyone)
