By filling out this form, you are requesting a facility access orientation tour for new or updated access to the LAR facility. Please submit one request per user.
Considerations:
- Once this form is submitted, it may take up to 1 week to coordinate the in person tour, and to grant necessary access. Please plan accordingly when preparing for your project.
- If you have been an inactive user for 12 months, your access will be revoked, and you will need to complete a new facility access orientation tour when needed.
- If you are beginning work in a new area of LAR or CPL, you will need to complete a new facility access orientation tour for that area before access can be granted.
Requirements for LAR Facility Access are as follows:
- Completed online General Orientation and Quiz (must be completed on the LAR Orientation Website before this form can be submitted)
- Listed on an approved IACUC Protocol to work unsupervised with animals. (For access for non animal use purposes, this is not required.)
- Completed the in-person Facility Tour for LAR, CPL, or RBV provided by the respective Managers or Supervisors of those areas. Tours are available on Tuesdays and Thursdays.
