This form is to record the service hours you have completed in the St Thomas community that will go towards your annual obligation.  

As a reminder, all families are required to volunteer a minimum of 20 hours per school year (July 1-June 30).  The only exception is those who paid for the buyout option.  40 hours are required for anyone who received financial aid. Every $20 equals 1 hour.
