As always, the safety of our members and staff come first. Central Park Tennis Club has put in place preventative measures to reduce the spread of COVID-19. However, Central Park cannot guarantee that you or your child(ren) will not become infected with COVID-19. By completing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by entering Central Park Tennis Club.

In consideration to Central Park opening in accordance with current regulations relating to COVID-19, I will not enter the Club if I have experienced any flu-like symptoms within the past 24 hours or had contact with anyone who has or is suspected to have COVID-19. I agree to take all reasonable steps to prevent my family and guests from entering Central Park if they have experienced flu-like symptoms within the past 24 hours or had contact with anyone who has or is suspected to have COVID-19.

I voluntarily agree to assume all risks and hazards incidental to such participation and do hereby waive, release, absolve, indemnify and agree to hold harmless Central Park Tennis Club, it’s instructors, agents, employees, directors, officers and persons for any claim arising out of any injury or illness to myself and/or my children. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Central Park Tennis Club, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Central Park program.
