Zion Lutheran Vacation Bible School Registration
This registration form registers your child/ren for VBS at Zion Lutheran Church in Kewanee, IL on July 18-22 (Sun-Thurs) from 6pm-8pm.  All information in this form will be kept confidential and by clicking submit on the last page, you are informing us that all the information you have provided is correct, and that you give permission for VBS leaders and Zion Lutheran to photograph/film the child designated above in any manner or form for any lawful purpose associated with the VBS program  (i.e. pictures during the event for publicity of the event, and for the next VBS).
