East Side Bike Club's E-Bike Library Participation Survey
Complete this form in order to receive more information about participating in East Side Bike Club's E-Bike Library. With this program, we are working with Shared Mobility Inc. (SMI) to test shared e-bike operations in Western New York, as part of a broader program that SMI has undertaken to demonstrate and plan for Electric Micromobility (EMM) in New York. East Side Bike Club's E-Bike Library will help model e-bike library programs to other groups in Western New York and around the country.

To participate in the pilot, you must:
- Be at least 18 years of age or older

- Be comfortable riding a bicycle on roadways with traffic in the City of Buffalo
I agree that I meet the above requirements for participating (age and comfortability riding a bicycle on roadways) *
To participate in the program and ride the e-bikes, you'll need to do the following:
1. Fill out this survey form

2. Fill out the pre-participation Electric Micromobility survey (part 2 of this form)

3. Attend a brief in-person orientation session

4. Follow all program rules while participating

5. Fill out the mid-program survey

Program details:
- Participation is free and includes 25 free e-bike rides. Participants will be asked to enter a credit card or debit card that will not be charged, except in the case of misusing/abusing the bikes.

- We’re testing this service and we need your input to make this a success. Again, this is a test. We're looking for participants that are interested in providing their feedback as we build this program together.

- Helmets must be worn at all times while riding the e-bikes. No exceptions.

- We're running this program with limited funding and staff time, we need participants to be partners with us in helping to make this pilot program successful.
