I agree to reimburse MMHA for all fines assessed by Hockey Saskatchewan for failure to obtain such certification. Certification requirements can be found on the MMHA website under Coaches Corner. I also agree to sign and abide by the Coaching Code of Conduct Agreement if selected as a coach in Moosomin Minor Hockey.

If you are a new coach to MMHA or your criminal record check on file is older than 3 years old you MUST provide a criminal record check that is less than 9 months old from the application submission date. Attach if applicable. CRC/VSCs are DUE BY OCT 15. * If there are delays with the criminal record check you can still apply but you must notify Secretary. Please contact secretary@moosominrangers.com if you require a letter to take to the RCMP.
