Field Trip: Visit to Advanced Micro Devices, Inc. (“AMD”) office located at 2485 Augustine Drive Santa Clara, CA 95054 (“AMD Site”) set to occur on July 14th, 2023 (“Field Trip Date”) for the CASPA Field Trip Event (“Event”).

In connection with CASPA field trip to the AMD Site on the Field Trip Date, you acknowledge and consent that You or Your child, as applicable, (referenced herein, as “You”) may enter the AMD Site and participate in the Event subject to the terms and conditions of this Permission form.

1. Access to AMD Site. You acknowledge and agree that you or your child , as identified below, will participate in the Field Trip and be granted limited access to AMD’s site during the Field Trip on the Field Trip Date subject to the supervision and requirements of AMD. You acknowledge and understand that You will have limited access to the AMD Site areas designated by AMD only and You will not be permitted to enter other areas of the AMD Site, and You agree not to take any photographs or recordings. In addition, during the Field Trip, You acknowledge and agree that You will comply with all applicable AMD policies and requirements. You understand and agree that in the event that You do not comply with AMD’s policies and requirements during the Field Trip or in AMD’s sole discretion, AMD may terminate access to the AMD Site upon notice to You.

2. Optional Server Room Access. Subject to You opting into the tour of the server room by checking the opt in box below, You acknowledge and understand that You will be given a tour of an active server room on the AMD Site. To enter the server room, You understand and agree that You will wear ear protection, which will be provided to You on the AMD Site, for the duration of Your time in the server room. You acknowledge and understand that the server room on the AMD site is an operating server environment that produces sound at high decibels. During Your time in the server room, You will be exposed to loud noise and heavy machinery and equipment. You acknowledge and understand that the server room is not a normal office environment and that You have been given an opportunity to opt out of visiting the server room.

3. Marketing Materials. Subject to You opting into being included in Marketing Materials by checking the opt in box below, you agree that AMD may use your likeness as set forth in this Section 3. For good and valuable consideration, receipt of which is hereby acknowledged, You hereby grant to AMD, its subsidiaries and agents, the unconditional, non-exclusive, perpetual, worldwide, royalty-free, irrevocable, license and right to use, reproduce, edit, alter, exhibit, make available to the public, publish, or distribute Your (or your child’s, as applicable) name, photograph, likeness, image, voice/audio recording, quotes, biographical information, interviews, statements, or any portion thereof (“Likeness”) for marketing and promotional activities in connection with AMD’s products and/or services, including not limited to, internal presentations, case studies, sales and trade show presentations, speeches, still photography, movies, videos, audio recordings, web content, advertising, annual reports, point-of-sale displays, and demonstration programs (“Promotional Materials”) in any media whether now existing or later discovered or invented, without further payment or other consideration. You agree that the Promotional Materials, shared and transferred internally to and from companies in the AMD group and to other third-party service providers in order to perform the services described above. You understand that where the Promotional Materials are shared with third party service providers that AMD will take steps to ensure Your Likeness is used for the specific purposes for which it was provided to them. You agree that no Promotional Materials will be submitted to You for approval, and AMD shall have no liability to me or anyone else for any distortion, illusionary effect or otherwise resulting from the use of Your Likeness. You agree that AMD and/or its agents own all copyrights to any Promotional Materials and to any derivative works from those Promotional Materials that AMD creates using the rights obtained by this Permission Form. You also agree that AMD, in creating Promotional Materials, shall have no obligation to identify You by name or otherwise, and You hereby release and discharge AMD, and its agents, representatives and assignees from any and all claims and demands arising out of or in connection with the use of Your Likeness, including without limitation any and all claims for invasion of privacy, misappropriation of the right of publicity, copyright or trademark infringement, and/or defamation. You understand and agree that personal information of Your child collected and will be processed in accordance with AMD’s Privacy Policy, available at: http://www.amd.com/en-us/who-we- are/privacy. You agree that Your personal information may be transferred and stored on servers outside my country of residence, which may not provide the same level of protection of Your personal information as in Your own jurisdiction and that it may also be processed by staff operating in the United States who work for AMD or for one of AMD’s third party agents or suppliers for the purposes outlined in this Permission Form. You agree to this transfer, storing or processing, and acknowledge that AMD is taking reasonably necessary steps to ensure that such information is treated securely and in accordance with AMD’s Privacy Policy.

4. General Terms. You agree that this Permission Form shall be binding upon You, Your child (as applicable), your heirs, personal representatives and assigns and shall be governed by the laws of the State of California and that proper venue for any dispute is Santa Clara County, California. You agree that this Permission Form constitutes the entire agreement between Yourself and AMD with respect to the subject matter within, and supersedes any and all previous agreements, whether written or oral, with respect to such subject matter.
