Please fill out an individual form for each player. We understand this may be time consuming and appreciate your cooperation. The safeguarding of your child is incredibly important to us, especially with contact sport. 

We will not share any of the details mentioned in this form with anyone outside of the GARFC volunteer team. Details shared within the team are on a 'need to know basis' only for safeguarding purposes.
