This Google Form is to be submitted by the Scheduling Support who is due for their Performance Review (you) prior to your In-Person Performance Review. Your Scheduling Supervisor or Office Manager will also complete an employee evaluation form. During the In-Person Performance Review, you and the Scheduling Supervisor or Office Manager will discuss where your evaluations align and where adjustments or further development would be beneficial.
You will be evaluating yourself on:
- Your exhibition of Forest Family Dentistry's Core Values;
- Your Scheduling Support Technical Skills; and
- Your Scheduling Support Soft Skills.
Additionally, you will share the professional and personal goals you want to achieve over the next year. At the end of your In-Person Performance Review, you and the Scheduling Supervisor or Office Manager will agree upon expectations for the following year to help you achieve your goals.
