Rush NY Future Prospect Form

IMPORTANT NOTICE: With the evolving Covid-19 pandemic affecting all sports organizations across the country, Rush NY is currently working on scheduling matters with all Rush Regional Directors and will communicate any Tryout, Summer Camps, and Player ID programs as soon as possible.

Thank you for your interest in Rush NY. We ask that you please, complete the following information below. Once, completed this will pre-register your son or daughter for an upcoming player evaluation event. This pre-registration will save you time and by-pass having to check-in and complete the form with one of our Rush NY event registrars.

If we have evaluations scheduled and you wish to attend the one specific to your age group, we recommend that you make every effort to attend multiple evaluation sessions. Attending multiple sessions will allow our professional staff to properly evaluate your child. Incase evaluation dates are not posted, you will be directly contacted via email by our Rush NY staff with date, time, and location of the event. It is important you complete this form and enter the correct contact information, as we would not want your child to miss out to be evaluated.
