THIS FORM IS A REGISTRATION/RISK DISCLOSURE FORM FOR ALL NMTBC DIG VOLUNTEERS.
At my request, Nelson Mountain Bike Club (NMTBC) will permit me to take part in voluntary mountain bike trail building activity. In consideration of being permitted to engage in mountain bike trail building activity, I agree to be bound by the terms, conditions and effect of this Agreement of Acknowledgment and Waiver Of Liability, Indemnity And Voluntary Assumption Of Risk (the ‘Agreement’).
I acknowledge and agree that:
1) I will attend a full safety briefing and I will personally perform a safety check of equipment in use prior to engaging in mountain bike trail building activity.
2) I am competent to safely engage in mountain bike trail building activity, and I will observe safe practices as advised and obey all instructions of the NMTBC team and experienced trail builders onsite.
3) I will wear all appropriate safety equipment at all times as advised.
4) I have represented that I am in good health and do not suffer from medical conditions which may be affected by engaging in mountain bike trail building activity.
5) I acknowledge that there are risks involved with trail building such as the risk of falling, use of trail building tools, other trail building participants (and their tools), hidden obstacles and varying terrain.
6) I have carefully read (or had someone else read to me) and understood this Agreement.
7) I authorise my name, voice, picture and information on this entry form to be used without payment to me in any broadcast, telecast, promotion, advertising, or any other way pursuant to the Privacy Act 1993. *
