HOW IT WORKS: Once you have filled out this form and submitted payment, I will contact you via email to set up a 30-minute consultation. During our consultation you can tell me everything I need to know about the special person in your life as well as ask me any questions you have. I'll write them a personalized song, I'll record it for you, and you can give them a copy as a gift!

WHAT IT COSTS/HOW TO PAY: The cost for a personalized song is $150 (Black Friday discount has been applied). At the bottom of this form, you will be asked to make your payment via PayPal or Venmo (if this is an issue, please email abbywalkermusic@gmail.com).

After you fill out this form and submit your payment, I will email you to set up a consultation time/date of your choosing.
