Items requested via Inter-Library Loan and items owned by USD's LRC are not eligible for delivery. Please tell us the location of your department's mail room including the building and room number. Items will be delivered to your department's mail room to be placed in your mailbox (not your office). Note: items cannot be delivered to Alcala Park West including Avila, Barcelona, Coronado, Durango.

I agree to renew or return all Copley materials loaned via department delivery on or before the due date.

I understand that faculty loan rules apply to all items I borrow using Copley's Department Delivery Service including fines and replacement costs for items not returned.

I understand that items ordered via Copley's Inter-library Loan Service are not eligible for delivery and must be picked up at Copley Library.

I understand that items will only be delivered on Tuesdays and Thursdays and may take up to 5 business days to arrive once requested.

I understand that once I enroll, all Copley items I put on hold using the online catalog's "Request It" feature will be delivered to my department within 5 business.

A copy of your responses will be emailed to the address you provided.
