Dear Parents or Guardians of Prisk Elementary Students,

The following is a digital version of the pink form sent to current Prisk families regarding your child's intent to attend Prisk next year!

If you already turned it in to your teacher before the school closure, it would be great if you could complete this form anyway; this way we have the information on hand and don't need to worry about collecting them whenever we return to school.

The last section of this form pertains to incoming kindergarteners.

If you have more than one child, please "submit another response" at the end of this survey.
