This registry is for individuals with an identical (monozygotic) twin.  If you have a fraternal (dizygotic) twin, this is not the registry for you; please see https://www.longhaulwiki.com/index.php/Currently_recruiting_studies for a list of studies that you could participate in.

If you would like to hear about future research studies looking to recruit you, please leave some contact information In the next question.  If you change your mind in the future, come back to this survey and fill it out again (but without your contact information).  This will remove your contact information from our survey responses.

WHAT WE WILL DO WITH YOUR INFORMATION
We will pass ALL of the information you provided in this form to research teams that are looking to recruit identical twins.  This means that other people will have your contact information AND your answers from this form.  If you are not comfortable with this, please do not enter in your contact information.

We will also publicly display your responses to the "Information that will be displayed publicly" questions.

Do NOT put in identifying information such as names, vaccine lot numbers, your address, etc.
