Welcome to the online application to serve in the Early Years Ministry, GraceKids, and GraceYouth at Grace Fellowship Church. We love our children deeply and are eager to provide a safe, secure and loving environment for them to grow and learn. To that end, we want to apply the highest standards of safety in this application process. This careful attention to each applicant is a demonstration of our desire to be above reproach and to ensure that families can entrust vulnerable children to our care.

This document is designed to collect information about you to qualify you for this service. Some of this information is quite personal, and we are committed to protecting your confidentiality. As such, this document may be read only by the person who provided us with the information (you), the elders, the GraceKids and Early Years Ministry coordinators, or other individuals specifically authorized by the elders to make decisions concerning who should serve in childcare at the church. No other person is authorized to review this document. Persons who review, duplicate, distribute, or disclose any portion of this document without authorization may face one or more of the following consequences: 1) Termination of employment, if an employee. 2) Dismissal from any official position within the church, if a volunteer. 3) Possible criminal liability. 4) Possible civil liability.

Before you proceed, here is the list of the more personal questions you will be asked to answer:

1. Have you been abused in your past in any way that would make it difficult for you to serve in our children's programs (sexually, emotionally or physically)?
2. Have you ever been accused of, participated in, plead guilty to, or been convicted of child abuse, child neglect, or any other crime against a minor?
3. Have you ever been convicted of or plead guilty to a crime (other than minor traffic violations)?
4. Have you deliberately and repeatedly viewed pornography in the past three years? (This includes reading, watching, or listening to, or in any other way using pornographic material, including books, magazines, television shows, movies, the internet, or telephone services.)
5. Do you have any ongoing sin struggles that you think would keep you from ministry to children?
6. As a child or teenager, did you ever have sexual interaction or contact with a child?

Choosing to not answer these questions does not automatically exclude someone from serving in this ministry.

If your past has included issues relating to abuse, you may want to talk to an elder or children’s ministry coordinator about whether this is a good time for you to apply and how Grace Fellowship Church can help you address these past issues.

If at any time you determine that the information you provided in this document is not true - or if it changes in a way that substantively impacts your qualification to serve in childcare, you are requested to notify the elders as soon as possible.
