Please complete this form in order to register your team for the Halswell School Quiz Night on 29 August 2020. Teams are 4-6 players, but we are able to allocate you to a team if you do not make up this number. If you have a team, a team name must be supplied. Please write "Allocate" in the Team Name field if you would like you and any others indicated on the form to be allocated to a team. Please also indicate how many tickets you are paying for.

We are not offering platters of nibbles to pre-purchase this year but small platters and snacks will be available to purchase on the night.

All payments are via Internet Banking and must be paid into the Halswell School PTA account.

Please use the reference QUIZ <TEAM NAME> or QUIZ <ALLOCATE>. If members of your team are doing their own payments, please pass the account details and reference information to them. They do not need to fill out a separate form.

Once payment has been received from all team members we will send out a confirmation email to the primary member whose email address is indicated on this form.

NOTE: We have a maximum limit of 30 teams. There are no tickets for this event. Teams will be checked in on arrival. Due to alcohol sales strictly no under 18.
