EQ PTA - Funds Requisition for Special Event
This form will be reviewed by the EQ PTA Board for agreement on disbursement of funds for the requested event. Amount of funds will be agreed and must be spent in accordance with the details of the form below. 

The submitter will work in coordination with the EQ PTA Board and EQ Elementary School to execute the proposed event. These funds are valid SOLELY for the 2023-24 school year; any intention for funding of a recurring event is subject to re-approval by the EQ PTA Board as well as the amount of funding available from fundraising efforts.
