The purpose of this survey is to understand the long-term effect of COVID-19 (known as Long COVID-19) as well as to study the side effects of coronavirus vaccines among ethnic minorities in the UK. We want to conduct the study because Long COVID-19 and vaccine side effects are under reported in ethnic minorities in the UK. The results of the survey will be shared with the public, and local and national health officials to enable them provide health care services tailored for ethnic minorities in the UK.

We will ask you questions about yourself, your health, COVID-19 related illness and coronavirus vaccination.

The survey takes about 5 min.

All information is anonymous and no person identifying information will be collected.
