2021-22 Basketball Registration
It is time to start planning for our Girls and Boys basketball seasons!  If your child(ren) is in grades 4-8 and would like to participate this year, please fill out this registration form before Friday September 2nd.  Please fill out once for each child participating.  The basketball seasons begins on October 3rd and will run into January.  The girls diocesan tournament is scheduled for January 6-8th and boys is scheduled for January 13-15th.

Basketball registration will be $60 per athlete.  Registration fees are due no later than October 3rd when the season officially begins.  Please make checks payable to the school that your child attends.  If we have late registrations or late payments, there will be an additional $10 late charge (per athlete) collected.  

 In order for your child to play, he/she must have an updated physical (valid for 12 months) on file in the school’s office.

Waiver I/We request that my child be allowed to participate in St. Joseph's Catholic School’s sports program for grades K through 8th. I/We assume responsibility for providing transportation to all practices and games. I have read the policies outlined in the Athletic Handbook. I/We further understand and agree that I/we assume full responsibility for any loss or damage to property, or bodily injury to others, caused by the child named below, whether by accident or intent. I/We agree that it is our responsibility to arrange for our child to be transported home at our expense in the event of violation of civil law or if deemed necessary by the designated trip leader.  I/We further authorize any emergency medical treatment, including hospitalization, that may be necessary as a result of any accident or injury arising from participation in the above activity and assume responsibility for payment thereof. I/We understand that this authorization is in advance of any diagnosis and is given as authority to render any treatment deemed necessary. *
