Please complete the form below by 12 noon on the Saturday preceding the Sunday class.

The information will remain confidential unless we need to divulge it to the Health Department for contact tracing.

Procedure for in-person Sunday school:
Students will have their temperatures taken by a teacher before entering the tent.
Students must sanitize their hands before entering the tent at the sanitizer station provided.
Students must wear masks that cover nose and mouth (neck gaiters are inadequate) for the duration of the class.
Physical distance must be maintained during the class.
Each child needs to bring a blanket or folding chair, as seating will not be provided.
Each child will be provided with their own stationary supplies in a zip lock bag labeled with their name.  Zip lock bags will be cleaned after every class and stopped at church.
