In this form we ask you to provide as much detail as possible regarding your more recent experiences with your NHS service providers including Hospitals, and briefly Doctors & Dentists.  

This initial Campaign is focused on Trusts with campaigners focusing on specific areas eg Mask wearing, Refused or Limited Patient Visiting etc

NHS staff are just as fed up with these measures and the friction it brings to their patient encounters, as we are.

Within the form, we also invite you to join our KBF Covid Enquiry.  We want to ensure the Public get the opportunity to TELL their MPs what their actions and inactions have led to, in terms of harm. You can share all the harms and suffering from Covid Policies, that you and yours have experienced since early 2020 in terms of Health, Mental Wellness, any Loss inc; People, Connections, Financial, Fitness, Business and more.

We will NOT share any of your personal or contact information with anyone else.  We will share ONLY the information that supports our Campaign to restore the services that your taxes have funded.

We want a Complete End To All Covid Measures.

✔ We want Empathy to return to any and all Patient and Family communication.
✔ We want full access to all treatments for all patients.

Your feedback on the challenges you have faced will help us build a picture of the Trusts that are following Government Guidance now vs those who are continuing to enforce anti scientific restrictions and policies onto the public.

We want to begin with the Trusts providing Hospital Services but in this questionnaire we have space to collect information regarding GPs and Dentists so we can store the information for later campaigns.

Some Trusts have restored compassion and will allow visiting and mask exemptions.  You are welcome to complete the form to show those Trusts you have positive experiences with.  We are looking for Trusts who respectfully accept any and all mask exemptions without questioning the patient or their relative.  They also must allow sensible patient visiting for the health and wellbeing of the patient and their family.  This must not be allowed on for end of life patients but for all patients regardless of their time in hospital.  We also want to hear if your Trust allows patients to refuse testing if asymptomatic and offer 'evidence' for any requests to perform these interventions.
