Diabetes Camp Involvement Form
Hello!  The Community Alliance for Progressive Education (CAPE) is very excited to have received grant funds from the Keweenaw Community Foundation to plan, develop, and implement a diabetes camp for young people in our community with Type I and Type 2 diabetes.   The grant objectives are as follows:

1.  Inform participants about diabetes; what it is and how it impacts the body.
2.  Provide interactive, hands-on activities to promote knowledge and habits in nutrition and exercise and how that supports managing the disease.
3.  Provide ideas and strategies to deal with chronic disease.
4.  Create a camp-like experience with crafts, and hikes, and other activities.
5.  Build community among participants that will lead to ongoing support strategies post camp experience.

If you are interested in being part of the planning process, there are three work groups you can choose to support:

Planning/logistics:  this group will be in charge of registration, tracking, preparing space and materials, communication among families, sending out registration, etc.

Research:  this group will research through multiple avenues the most up to date health and medical information regarding Type I and Type 2 diabetes to share with families at the camp.  There may also be research into devices, activities and grants.

Curriculum/Camp experience:  this group will be focused on developing the camp experience to ensure it is engaging, hands- on, meets the grant objectives and is fun.

Please plan on meeting every 2 weeks with your workgroup via zoom.  The dates and times for those meetings will be set by the members of that work group.  You are welcome to join as many workgroups as you wish.

SUSAN NIELSEN is the overall facilitator of the process to achieve success through the planning and development process as well as execute the camp experience and the follow up.

THE TENTATIVE DATE FOR THE CAMP IS JUNE 17TH TO 21ST.
