All public colleges/universities in Louisiana have access to your transcript through the Student Transcript System and we do not have to mail them.  If you will attend an out of state or private college/ university, your transcript must be mailed.  Please complete all fields below. You must include your receipt number from on locallevelevents.com in order to complete the request. Cost is $5. Students who graduated prior to 2000 should contact the records department at the Caddo Parish School Board. Students applying through the Common App should not complete this form.
