Please fill out all answers below. Completing and submitting this form is a commitment of attendance. If you are unable attend, call SOMT Sports Director, Scott Held IMMEDIATELY at 406-216-5327.

The maximum number of registrants accepted for this training is fifty (50). Registrants will be accepted on a first-come, first-serve basis, so if you would like to commit to reserving a spot, complete as soon as possible. A new registration must be completed for every individual wishing to attend.

SOMT will make room reservations. The plan is to offer rooms at reduced costs for coaches willing to room with another coach of the same gender. See below. Breakfast at the hotel is included in room reservation.

I would like to reserve a single bed in a double room. Unless I specify the name of a coach I'd like to room with, who will also be attending the training, I will accept a random roommate assigned to me by SOMT. By choosing this option, SOMT will cover a portion of the room costs, and will bill my team for the additional cost not covered.
