Throughout the Marching Band season, photographs or video may be taken of the students for purposes
including, but not limited to, Farmington Public School District and NFHS Band & Orchestra Boosters
websites, social media and publications, video productions, and news media.

I agree to absolve, release, and hold blameless Farmington Public Schools, North Farmington High School, the directors, or other adults participating in the trip/camp, from any financial liability or claim for damages of any nature arising out of the events associated with the North Farmington High School Marching Band & Orchestra. I realize such experiences have certain risks involved and that every attempt will be made to safeguard students and equipment.

In compliance with the Americans with Disabilities Act, the North Farmington High School Band and
Orchestra program will not discriminate against any individual on the basis of physical or mental
impairment with respect to participating in the organization’s activities. The North Farmington High
School Band and Orchestra program will make reasonable modifications in policies, practices and
procedures to accommodate the needs of individuals with physical or mental impairment, unless the
modifications would fundamentally alter the nature of its activities.
