As COVID-19 continues, Millbrook Youth Hockey Association (MYHA) will periodically update our policies & procedures based on current recommendations from the CDC and health and safety authorities in local governments.

MYHA requires health confirmations from every player prior to every team event. To avoid having to fill out this form every time,  the statements in this self-screening form and waiver will be deemed to have been made EACH TIME you RSVP “yes” to a team event (game, practice, etc).

Players will not be permitted on the ice or in the rink if they have not RSVP’ed “yes” on the website or app for that specific practice, game or other team event.

This form is for Millbrook Youth Hockey association, you will likely need to fill out a separate form for the rink.
