Hello Confirmation Teens!  Please complete the following form with all of the important information that we need to know prior to you making your Confirmation.  This needs to be completed by March 22nd.

Each Confirmation Candidate must select a Confirmation name.  This name can be your baptismal (given) name, or the name of another Catholic saint.  In the Bible, a change of name often signifies a conversion or a new sense of mission (i.e. Saul to Paul).  Thus, the selection of a Confirmation name represents not only a new sense of mission in the Church, but the recognition of another saint whose example is one that is admired.

*Please note, there are requirements for being a sponsor.  A sponsor is someone who is a Baptized, Confirmed Catholic who attends Mass regularly and whose life bears witness to the catholic faith. A sponsor may not be the father or mother of the individual, but may be any other person who meets the aforementioned criteria. (Canon Law 892,893)
