I hereby acknowledge that I have reviewed the current year Dixie AFJROTC Booster Club Handbook.  I understand the content contained within the handbook and agree to follow the guidelines to the best of my ability.  Any questions I may have must be directed to the Booster Club President.  

I understand that if I chose to chaperone an event, the proper protocol must be followed, as outlined in the handbook.  I will communicate with the appropriate chain-of-command when I have questions or a situation arises.  As a chaperone, I am an extension of the cadre and agree to uphold all standards set forth for the cadets. 

The objective of the handbook is to ensure information is communicated consistently to all Booster Club Members to effectively assist the cadre to run the AFJROTC program as seamlessly as possible.

The completed form is kept on file by the Booster Club President for the current school year. A new form is completed by the Booster Club Member annually.
