The University of Maine system is no longer asking students to disclose the following information when applying to attend university, however the following conduct questions do need to be answered by all students participating on any activity that requires a residential or over-night accommodation, which includes study away participation per UMS policy.

I permit the Office of International Programs to obtain, for the purposes of this application, any and all education and student conduct records at USM or any other institution which I've attended on a full‑time basis. I understand that, once accepted into the program, I may be required to pay a deposit that will not be refunded if I withdraw from the program for any reason. I also understand that USM reserves the right to cancel this program due to low enrollment, changing political or economic factors, unusual circumstances, or any other event which would dramatically impact the nature of the course or the ability of the faculty to conduct such a course.

I affirm that all the information and answers to questions in this form are complete, true and correct to the best of my knowledge and belief. I understand that any misrepresentation, falsification, or omission in response to any questions may render this form void and may be cause for cancellation of my participation, whenever discovered.
