Starting School Information
This form is for parents/carers of children that have been offered a place by Blackpool Council to start school at Layton Primary in September.  It is important that this is completed prior to your child starting with us.  We also need a copy of your child's birth certificate, you can email a photo to admin@layton.blackpool.sch.uk.  Alternatively, we can collect a copy another way.

The form has some 'required' questions, if these are not applicable to your child, please type 'N/A' or 'none' to allow you to move to the next question.
