Please complete this form for each student in your family who might desire busing from your home district next year. I cannot guarantee that you will have busing, but I would like to have information and data to share with the school districts, so that they can make an informed decision about providing busing for next year. As of right now, Zane Trace, Chillicothe, and Southeastern provide busing. Everyone else provides payment in lieu of busing. If you think that you might need busing, please indicate this on the form.

PLEASE BE AWARE THAT YOU PERSONALLY NEED TO REQUEST BUSING FROM YOUR SCHOOL DISTRICT OF RESIDENCE. We can make them aware of dates and people who may want transportation, but if you do not contact the district, you may lose the chance for busing.
