Sport Physiotherapy Canada Credential Program Mentors application. Please complete the following form if you wish to be an SPC Credential Program Mentor.

Please remember to be approved you must meet these requirements:
  • Be a current SPC member;
  • Have up-to-date Maintenance of Credentials (MoC) for either the Certificate or Diploma designation;
  • Registered for the Core Competency Courses if their last MoC is before 2023.
