I understand that I will have to pay a participation fee in case of any extra administrative work of an organizations such as selection process, consulting, preparatory visit, assistance.

By submitting this application I acknowledge the following conditions:

1. I will come fully prepared (will read infopack and materials sent by organisers). 

2. I commit myself to participate in the whole process, including: to prepare myself carefully for the project; to do all remote preparation work the team will ask for; to take part in the full duration of the project; to participate in the whole evaluation process.

3. I acknowledge that this project is a learning process and not a holiday, therefore I am committed to work on my development and manage my own learning, supported by the facilitators and peers. Reimbursement will take only if I participate in full duration of the programme.

4. I commit myself to participate in the whole process and I will take part in the all project activities: preparation, participation, filling the evaluation form and disseminate the results.

5. I understand that the information provided on special needs does not remove my personal responsibility for ensuring my own health.

6. I am aware that my traveling tickets to/from the project have to be the most economical. Reimbursements will be sent as soon as possible, once (and if) I take part in full duration of the programme, fill the evaluation forms, do the disseminations, present all the traveling documents, including boarding passes.

7. I acknowledge that in case I don't follow all the conditions stated above I may be asked to leave the project before its completion.

8. I confirm that during the project i do not receive other Erasmus+ funding. During the study project I am not participating in any other Erasmus+ project (Student exchange, ESC or Erasmus Intership). I am aware that participation in another project activity at the same time is double funding and it may cause financial implication to the organizers of both projects.

9. I give the full consent to be photographed and filmed during the mobility and that the photos and videos taken to be used publicly by the beneficiary organisation, partners, National Agency and EACEA. I am informed that my sensitive personal data and images such videos & photos of me can be used in printed and digital formats during & after the project. I give my permission for using photos and videos with me made within the project for project dissemination. I do not claim any right for the photos and videos taken during the project. 

10.In compliance with the General Data Protection Regulation (GDPR, 2016/679), I hereby authorize you to record, store and process my personal details contained in this application form. I give the full consent to provide and to allow the processing and storing of all the personal data required and for the implementation of the Erasmus+ project, in accordance with the European Commission regulations. The data will be stored and processed by the Programme's beneficiary organisations and the National Agency under the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation - GDPR). The National Agency will store and process these data according to Regulation (EC) No 45/2001 of the European Parliament and of the Council of 18 December 2000 on the protection of individuals with regard to the processing of personal data by the Community institutions and bodies and on the free movement of such data.
