Please be advised of a few important things as you fill out this form:

(1) We are not currently in an active provider-client relationship in which you have signed consent forms, etc. So I am not able to respond to/provide therapeutic support for the answers you provide on this wait list form. So please use your discretion in terms of the information you feel comfortable disclosing to me.

(2) I cannot guarantee that, by filling out this form, you will be able to get a therapy spot. It is therefore advisable that you continue your search to make sure you get the support you deserve.

(3) Should you experience a mental health crisis while you are on this waiting list you should either call 911, or go to the nearest hospital emergency room.

(4) Many people are on multiple waiting lists for therapy, and it is completely acceptable for
you to be on a few waiting lists at a time. If you start services elsewhere before you possibly get a session slot with Clare, that is totally fine! You can either try to remember to update your wait list response or just let Clare know that you found someone else to work with if she reaches out to you.

(5) There is no penalty or consequence for requesting services with Clare again if you have been on their waitlist before.

My current practice schedule is the following:
 
Mondays  2pm - 5pm*
Tuesdays 10am - 5pm
Thursdays 2pm - 5pm*
Fridays 10am - 5pm

*My KAP (Ketamine-Assisted Psychotherapy) slots always occur on Mondays from 10am - 1pm and Thursdays from 10am - 1pm.

Considering this schedule, please list your current availability for sessions. You can state "flexible/open," or provide specific days/times that you can schedule sessions within this practice schedule.

You may have seen from my bio that I practice in certain areas. If you feel comfortable, please check off any of the issues/needs that you're looking for support on:
I have shifted my practice to only providing short-term (3-6 months) sessions that utilize a combination of EMDR, IFS, somatic, and KAP modalities for trauma reprocessing and healing.

I am no longer offering long-term therapy or therapy that is primarily talk-therapy based.

I have found that short-term treatment can be particularly effective if the person is already working with, or is willing to work with, an ongoing therapist that I can consult and collaborate with (always with your consent). 

You are more likely to achieve gains with EMDR or KAP if you are able to continue to explore and integrate the outcomes of these treatments with your ongoing/longterm therapist.
