2024 CAM SCHOLARSHIP APPLICATION

Eligibility Requirements:
  1. Resident of Dane County or attending/planning to attend a college or university in Dane County
  2. Community Involvement (e.g. volunteering, activism)
  3. Demonstrated Financial Need
  4. Academic Achievement
  5. Need two reference letters (1) personal(non-relative) (2) educational
Instructions:
  1. Complete this application form in its entirety.
  2. Complete a separate Personal Statement and save it as: "LastName, FirstInitial - CAM Personal Statement"
  3. Complete a separate statement of financial need and save it as: "LastName, FirstInitial - CAM Financial Statement"
  4. Email your Personal & Financial statements to camscholarshipawards@gmail.com by the application deadline (May 31st, 2024 at midnight CST).    
  5. Have two references email their letters of recommendation directly camscholarshipawards@gmail.com, with your name in the subject line, by the application deadline (May 31st, 2024 at midnight CST).
