In this year’s program, I/we agree to discuss these rules with my/our team and to follow all rules and regulations. I/we understand that the failure of any member or affiliate of my/our team to adhere to the rules may result in disqualification of the team. *

I/we understand that this signed teacher-coach(s) agreement/entry form and payment of $800 is due by October 10 to enter into this year's competition. Additionally, all registration (student permission slips, FCSS Registration, and CRF Registration) is due December 8th.  If these forms are not received, my/our team may be disqualified. *
