Form filling Guidelines:

  before filling the online form ( Registration
  requester will email complete filled and duly signed scanned pdf copy at msmvsports@gmail.com)
       
2.    Online form is only for registering the team for tournament and registration fee deposit.

3.     After registration is confirmed from the officials, each player of team is required to submit the physical copy of form
        latest by 1st October 2019

4. Team registration fee for tournament is  Rs. 3500/-  (Non-refundable)
