Some things to remember as your are filling out the survey.

Food must be picked up by the child or a parent/guardian of the child.  Older siblings can pick up for the household where they reside.  Child care centers can not pick up for the children in daycare.  We will be offering 7 day meal packs.  A 2nd survey will come out closer to summer for you sign up for the weeks that you will be wanting to pick up food.
