Student Transportation Request Questionnaire    
The main goal for the student transportation questionnaire is to collect the necessary information from the students who will need transportation to and from the Perry Career Academy Main Campus. Please note that once the information has been verified by the Transportation Department the student will be routed for the PM route that best fits the home address. Transportation closes each week at 12:00 PM on Wednesday. The students who have filled out the survey by then will be routed for the following Monday.
