This form should be submitted in conjunction with the Lottery Application to PPS, Enrollment & Transfer during the Lottery window of November 5th, 2019 at 8 am, to December 3rd, 2019 at 5 pm. This form WILL NOT enter your student in the lottery; it is only to provide information about da Vinci so that applicants have a better understanding of our program. Our hope is that students entering the lottery have a sincere desire to attend da Vinci and families understand the expectations for students and families. There are two information sessions scheduled before the lottery application deadline. Attendance is encouraged but not required.
