Please complete a separate questionnaire for each child that attends Colonial Elementary.

A staff member will email you with the date of when your child's isolation or quarantine ends, the process for remote learning and when they can return to school. You can expect an email within 24-48 hours or the following Monday if you complete this questionnaire on a Friday, Saturday or Sunday.
