Fantasia Music School: Registration
In 2022, Fantasia will run from August 21st–26th.

Residential students are expected to arrive on the afternoon of Sunday 21st and leave after the student concert on the afternoon of Friday 26th. Day students are expected to attend all day from Monday 22nd to Friday 26th.

Please state how long (in years and months) the student has been studying each instrument, and (if appropriate) their current Grade and the date the Grade was passed. (Note: 'Grade' refers to the system of English music exams, ABRSM or equivalent). If your child does not take graded exams, please give any information you can about their proficiency. Please list all instruments that the student will bring and expect to play on the course.

MEDICAL INFORMATION
We will discuss and confirm any important medical information relating to the student upon arrival, but it is helpful to us for you to provide any relevant medical information now that you think could affect their participation in any way — specifically allergies, chronic conditions or disabilities.
Emergency contact numbers
Please provide two emergency contact numbers and state relationship to student. These numbers will only be used in the event of a medical emergency.
Doctor’s name
Please provide the student’s doctor’s name, address and telephone.
Ongoing medical conditions & medication
Please provide details of all current or ongoing medical conditions and medication. On arrival, please make sure to discuss these details in person with our staff. All medication or medical equipment must be handed in on arrival to our registered First Aider. These will be returned to the student as necessary.
Allergies
Please include a complete list of any food allergies or special dietary requirements, if necessary. On arrival, please make sure to discuss these details in person with our staff.
PAYMENT INFORMATION
Once we receive your registration details, we will contact you by email or phone (using the contact information you have provided above) to confirm your registration and arrange payment. For registrations made before the end of April, full payment is then due in full within 30 days of confirmation. For ‘late’ registrations (May—August), payment is due within 7 days of confirmation. Please see our website for details of our family discounts.

In the hopefully unlikely event that new COVID-related government advice means that we are unable to run Fantasia as planned, all fees will be refunded in full.
