Village Primary Care in partnership with Waukesha County Public Health Department, and the Wisconsin Department of Health Services, to offer FREE community COVID testing in the Moreland Medical parking lot located at 1111 Delafield St, Waukesha, WI.

Testing is by appointment ONLY for drive-up testing. Testing is available to anyone age 5 or older. Appointments are listed for one week at a time. Future testing dates will be added to this sign-up weekly.

Registration is also required at: https://register.covidconnect.wi.gov/en-US/  Please complete this registration form in addition to setting up this appointment.  You will receive an email after registering that contains a QR code.  Bring that QR code with you to your testing appointment.
