Thank you for completing this form, which will assist in preparing for and running our Annual Vestry Meeting on Zoom.

Please note that:

You are asked to complete one registration form per voting member of each household, and to use the same name on Zoom.

We will be recording votes using Zoom polling technology. A motion or question will appear on screen and you will be asked to respond. If two members of the same household are on the same computer or smartphone, one member will respond to the poll, and the other will use the chat function to respond.

Zoom does allow for phoning in, but please note that it is not a toll-free number, and long distance charges may apply. If you dial 10-10-710 before the Zoom phone number, your long distance charges will be reduced to $1/hour.

An agenda, vestry reports, motions and other meeting materials will be circulated by email prior to the meeting.
