Ministry of Health COVID-19 Booster Registration
Persons who have received 2 doses of a COVID-19 vaccine more than 6 months ago are eligible for receiving a Pfizer-BioNTech Booster Dose.  Boosters will be offered to persons who are:
- 50 years and older
- Immuno-compromised
-Persons with underlying health conditions associated with high risk of COVID-19 complications e.g. Diabetes, Hypertension, Heart and Lung disease etc.
- Healthcare workers
- Residents of elder care facilities

Persons interesting in registering for a booster dose are urged to complete the form below. Please provide  current contact information as you may be contacted on short notice.
